Pour the cooled coffee and 2 cups of ice into a blender.
Add the milk, caramel syrup, and sugar to the blender.
Blend everything together until the mixture is smooth and has a frothy texture.
If it’s too thick, you can add a splash of milk to thin it out.
Pour the frappe into a large glass or two separate glasses, depending on your serving needs.
Top off with a generous amount of whipped cream.
